Meredith Grey, the daughter of the once-renowned surgeon Ellis Grey, becomes an intern at the Seattle Grace Hospital. She meets fellow interns Cristina Yang, Isobel "Izzie" Stevens, George O'Malley and Alex Karev who become her closest friends during the intern program. She finds herself working with renowned surgeons such as the heart surgeon Preston Burke and Richard Webber, chief of surgery and old friend of her mother. She finds herself working under her one night stand and famous neurosurgeon Derek Shepherd and working with her resident Dr. Miranda Bailey also known as The Nazi.

During an interview, Shonda Rhimes stated that "Season 11 is really a Meredith-centric season. She lost her ‘person’, her half-sister has shown up, her husband is chafing to go someplace else…" She went on to reveal that she's been wanting to do the "familial grenade" storyline for a long time, and at the end of Season 10, she knew it was the time to do it. Rhimes also claimed that Season 11 will pick up right where Season 10 left us, so there won't be much that the audience won't see. In another interview discussing this storyline, Rhimes revealed that she and the writers are thinking about doing flashback periods to the younger days of Drs. Ellis Grey and Richard Webber.

The doctors find themselves simmering in a world of sexual politics. After being sick with COVID-19 for weeks, Meredith split her time between two hospitals. She was hired by Dr. Hamilton to work on a cure for Parkinson's in Minnesota. There, she reconnected with Nick Marsh and began dating him while still working part-time at Grey-Sloan in Seattle. She decides to move her family to Minnesota with Nick after helping Grey-Sloan gain back its accreditation for the residency program that was on probation.

Now in its 19th season, a lot has changed around Grey Sloan Memorial. Following Bailey's resignation, Meredith assumes the role of Interim Chief of Surgery; under her tenure, she has revived the residency program, recruiting surgical residents who either were not good enough to get into other programs or kicked out of their previous ones. As the team of doctors seek comfort from one another, they discover that neither medicine nor relationships can be defined in black and white.

The lights are still on at Grey Sloan Memorial Hospital as the twentieth season returns. In response to Catherine's ultimatum, Meredith resigns from the Fox Foundation and instead publishes the research publicly, valuing its potential to save lives over hospital politics, and departs Grey Sloan while discussing a future together with Nick. In retaliation, Catherine fires Teddy for secretly funding Meredith's Alzheimer's research. Winston faces relationship doubts after Maggie cancels plans for him to visit Chicago multiple times.
